Program

Alumni will participate in an interactive case discussion focused on applying the Geriatric 5Ms framework to their specialty or setting of practice.

About the 5Ms of Geriatrics 

Published in 2017, the Geriatric 5Ms (Tinetti et al., JAGS 2017) were created to explain the geriatrics field and align with the Age-Friendly Health Systems Initiative, in which several HMS-affiliated hospitals are participating. The framework centers on the five main domains of caring for older adults from an interprofessional perspective: 

  1. Multicomplexity: Describes the whole person, typically an older adult, living with multiple chronic conditions, advanced illness, and/or complex biopsychosocial needs.

  2. Mind: Consider mentation, dementia, delirium, and depression.

  3. Mobility: Consider the amount of mobility and function, impaired gait and balance, and fall injury prevention.

  4. Medications: Consider polypharmacy and deprescribing, optimal prescribing, adverse medication effects, and medication burden.

  5. What Matters Most: Consider each individual’s own meaningful health outcome goals and care preferences.

Andrea Wershof Schwartz, MD, MPH, is an associate professor of medicine at Harvard Medical School, where she co-directs the Medical Education Course for medical students and the Aging and End of Life Care Theme. She is also an assistant professor in the Department of Epidemiology at the Harvard T.H. Chan School of Public Health, a geriatrician, and the associate director for clinical innovation at the Veterans Affairs Boston Healthcare System.
