Participate in this time-honored tradition and submit your entry online or via email. Reunion Reports are an integral part of your HMS/HSDM legacy, providing a fantastic opportunity to share your story and reconnect with your classmates.
NEW THIS YEAR
- We automatically print names, nicknames, societies, specialties, job titles, and employers that we have on file, but in order to include your contact information, personal updates, photos, and a letter to your classmates, you must provide consent when you submit your entry.
- To receive a printed copy of your book, you must confirm your mailing address when you submit your entry. Reunion Reports will be mailed in May.

Your HAA ID number can be found in the footer of recent emails from our office and the Harvard Alumni Association, and your preferred email address on file is the address that received the email. If you need help, contact the Harvard University IT Support Desk at 617-495-7777 or ithelp@harvard.edu.

- Save the questionnaire to your computer. You can type your responses directly into the PDF. You can also print the form and handwrite your responses.
- When you’re finished, save your completed form or scan or take clear photos of your handwritten submission, and email it to hmsalum@hms.harvard.edu.
